چکیده

OBJECTIVE Measurement of arterial stiffness (AS) is still hampered by the technical skill necessary to obtain reliable measurements. It is generally considered that AS is not prone to self-measurement. We developed a non-invasive technique to assess AS from a connected bathroom scale, based on ballistocardiography (BCG) and impedance plethysmography (IPG).(Figure is included in full-text article.) DESIGN AND METHOD :: We included 76 healthy subjects and 35 essential hypertensive, 50% women. The scale includes high sensitivity transducers measuring oscillations of weight due to cardiac contraction (BCG). BCG signal comprises G, H, I, J, K, L, M waves. The HIJ waves are hallmarks of systole and were subsequently tracked. The scale comprises electrodes to measure the blood flow in a foot (IPG), used as a hallmark of flow wave arrival in the foot. The scale pulse transit time (WS-PTT) was calculated by linear transformation of the time intervals between the BCG systolic waves and the foot of the IPG. Several estimates where combined to improve the robustness of the measurement in a home setting. Carotid to femoral transit time (CF-PTT) was measured using Sphygmocor (Sydney, Australia). Data are presented as mean ± SD (min-max). Spearman and robust multivariate regressions were used. RESULTS The age was 45 ± 17 years (20-75), SBP 125 ± 18 mmHg (92-174), CF-PTT 66 ± 15 (28.3-100.9). The WS-PTT correlated well with CF-PTT with R = 0.62. The root mean square error was 8 ms, and the slope of the correlation significantly differed from 1 (0.73, 95% CI [0.62 to 0.84]), indicating a significant bias. Correlations of WS-PTT with age and blood pressure were similar or stronger (R = 0.73 and 0.37, resp.) than CF-PWV (R = 0.48 and 0.37, resp.). This good correlation is non-trivial given the differences in wave paths, the fact that measurements are made in orthostatic position and totally investigator-free. CONCLUSIONS A simple user-oriented instrument such as a connected bathroom scale can estimate arterial stiffness with accuracy close to healthcare-oriented systems. The availability of arterial stiffness data on very large, non-medicalized populations will change our management of well-being and health.

Measurement of Aortic Pulse Wave Velocity With a Connected Bathroom Scale

BACKGROUND Measurement of arterial stiffness should be more available. Our aim was to show that aortic pulse wave velocity can be reliably measured with a bathroom scale combining the principles of ballistocardiography (BCG) and impedance plethysmography on a single foot. METHOD The calibration of the bathroom scale was conducted on a group of 106 individuals. The aortic pulse wave velocity w...

Validation of the noninvasive assessment of central blood pressure by the SphygmoCor and Omron devices against the invasive catheter measurement.

BACKGROUND To investigate the accuracy of the SphygmoCor and Omron HEM9000-AI devices in the estimation of central blood pressure (BP) in comparison with the simultaneous invasive catheter measurement. METHODS The radial arterial pulse was sequentially recorded by the use of the Omron and SphygmoCor devices in 33 patients, with the calibration of the brachial oscillometric BP, to derive centr...

Assessment of arterial stiffness in hypertension: comparison of oscillometric (Arteriograph), piezoelectronic (Complior) and tonometric (SphygmoCor) techniques.

BACKGROUND Arterial stiffness, measured as aortic pulse wave velocity (PWV), and wave reflection, measured as augmentation index (AIx), are independent predictors for total and cardiovascular morbidity and mortality. The aim of this study was to compare a new device, based on oscillometric pressure curves (Arteriograph), which simultaneously measures PWV and AIx, with standard techniques for me...

A new oscillometric method for assessment of arterial stiffness: comparison with tonometric and piezo-electronic methods.

INTRODUCTION Pulse wave velocity (PWV) and augmentation index (AIx) are parameters of arterial stiffness and wave reflection. PWV and AIx are strong indicators for cardiovascular risk and are used increasingly in clinical practice. Previous systems for assessment of PWV and AIx are investigator dependent and time consuming. The aim of this study was to validate the new oscillometric method (Art...

Presentation of a Non-invasive Method of Estimating Arterial Stiffness by Modeling Blood Flow and Arterial Wall Based on the Determination of Elastic Module of Arterial Wall

Introduction: Arterial stiffness is an important predictor of cardiovascular risk. Several indices have been  introduced to estimate the arterial stiffness based on the changes in the brachial blood pressure. Since the  substitution  of  the  blood  pressure  changes  in  the  central  arteries  such  as  carotid  with  the  blood  pressure  changes in the brachial results in error in the blood...
